*General Summary:* Under the supervision of the ADHC Director, the Health Care Associate is responsible for working with indirect supervision from the Clinical Supervisor.
Responsible for providing and assisting participants of Horizon PACE with activities of daily living, restorative and supportive care.
Assists with organization and implementation of activity program.
Monitors health status and provides a safe environment for participants.
Functions as a member of Interdisciplinary Team.

A are current license as Certified Nursing Assistant in the State of Kentucky is required.

*Experience:* A minimum of one year’s of experience in working with the frail or elderly population preferred.

*ESSENTIAL FUNCTIONS*: 1.
Assists participants and or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
2.
Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Director of Activities.
3.
Monitors and assists participants with meals at feeding table.
4.
Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.
5.
Accompanies enrollee to medical and other appointments.
6.
Performs and or assists with laundry, including clothes for participants.
7.
Assists the Drivers with the loading and unloading of participants from vehicles.
Travels with Drivers to act as an escort for participants as needed.
8.
Demonstrates dependability through consistent compliance with scheduled work hours.
9.
Participates in the activities of the Interdisciplinary Team as needed and carries out the plan of care as directed.
10.
Notes changes in participant’s physical or emotional health and communicates this information to the appropriate staff person.
11.
Maintains participants’ medical record and fulfills Horizon PACE charting and reporting requirements.
12.
Assists the Transportation Department with participants’ takes home supplies.
13.
Participates in and support Quality Improvement Initiatives.
14.
Follows all Horizon PACE program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
15.
Acts within the scope of his or her authority to practice.
16.
Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
17.
Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
18.
Maintains safe working environment.
Follows Horizon PACE Safety policies and procedures.
19.
Participates in continuing education classes and any required staff and training meetings.
20.
Performs other duties as required or requested.
*GENERAL RESPONSIBILITIES* 1.

Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment.
3.
Supports, cooperates with and implements specific procedures and programs for: a.
Safety, including standard precautions and safe work practices, established fire/safety/disaster plans, risk management and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
b.
Confidentiality of all data, including participant, employee and operations data and reports breaches immediately to the HIPAA Privacy or Security Officer.
c.
Quality Imorovement and compliance with all regulatory requirements.
d.
Compliance with current law and policy to provide a work environment free from harassment and all illegal and discriminatory behavior.
4.
Supports and participates in common teamwork: a.
Cooperates and works together with all co-workers; plans and completes job duties with minimal supervisory direction, including appropriate judgment.
b.
Uses tactful, appropriate communications in various situations.
c.
Follows up as appropriate with supervisor, co-workers or participants regarding reported complaints, problems or concerns.
d.
Promotes positive public relations with participants, caregivers, and guests.
e.
Adheres to attendance policies, including clocking in and out for meal time, following break schedule, and seeking supervisor’s approval prior to working overtime.
f.
Completes requirements for in-service training, uniform and dress codes including personal hygiene, and other work duties as assigned.
5.
Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
6.
Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
7.
Assists participants and caregivers in filing grievances.

*Visual, Hearing and Communication Requirements* - Requires corrected vision and hearing to normal range.
Must be able to communicate verbally with all staff, caregivers, participants, and community at large.

Moderate pressure to meet scheduled appointments.
Subject to people that may have the potential for verbal or physical aggression.

*Physical Requirements* – Shall be in good mental and physical health, and capable of performing assigned tasks.
Employee positions require strenuous activity including pushing, pulling, lifting of 40 pounds or more, standing for long periods, extensive walking, and movement at different position levels.
